Medical Screening Questions

To establish a safe and healthy environment for all participants in its boarding program, Texas West Animal Health (TWAH) requires that all dogs have up-to-date proof of Rabies, DHLPP or DAPP, Bordetella, and a Negative Heartworm Test and be on current heartworm prevention.

In the event your pet is due for vaccine(s) or requires a booster, the vaccine(s) must be administered at admission to the boarding facility and applied to the boarding invoice. If your pet was seen at another clinic, we must receive a copy of the medical records or vaccine certificate.

If your pet has fleas or ticks at admission, a dose of NexGard or Bravecto will be given to kill all external parasites and protect other pets in the facility, at your expense.

Pets that are under the age of 12 weeks are required to have completed 2 rounds of DHPP and Bordetella. If not completed, they will have to stay at the clinic's day kennels.

I verify that the above-named dog(s) are in good health and, to my knowledge, have not shown clinical signs of any communicable disease within the last 14 days. I further certify that my pet(s) have not caused harm or shown aggressive or threatening behavior towards people.
